We pride ourselves on running on time and we will promptly let you know if we are not. We kindly ask that you return the favor. If rescheduling is necessary, please allow plenty of advanced notice as we may have someone needing your time slot.

• Patient Medical History Form
• Insurance Card
• Referral Form
• Co-Payment

Our office is contracted most health plans and insurance companies including but not limited to:

• Medicare and Medicare Advantage Plans
• HMO’s thru Meritage Medical Network
• PPO Insurance Companies – Blue Cross; Blue Shield; Anthem; Aetna; Cigna; HealthNet; United Healthcare; and many more
• Affordable Care Act Plans
• Third Party Commercial Insurance companies (TPA’s)
• We are also listed on several Workers’ Compensation MPN’s.

To inquire about whether we are on your specific insurance plan please contact us and we will be happy to inform you if we are contracted with you plan.

Please remember to bring your insurance information with you to your appointment. We can only bill your insurance and seek reimbursement from them if we are provided valid information for each visit.
